CHRONIC CARE MANAGEMENT

What is Chronic Care Management?

Managing long-term health conditions can be a complex journey, but you do not have to navigate it alone. Chronic Care Management (CCM) provides a dedicated support system to help you maintain your wellness, achieve your health goals, and stay active for the moments that matter most.

Care Managers link patients with their medical team, making sure important health updates are communicated between patients and healthcare providers.

​

 

They keep the Valley View Health Center informed, allowing for quick changes to treatment plans to prevent serious problems.

This medical program is designed to assist you in managing chronic health conditions, including:​

  • High Blood Pressure

  • High Cholesterol

  • Diabetes

  • Lung Disorders

  • Heart Conditions

  • Memory Loss

  • Seizures

  • Acid Reflux

  • Depression

  • Anxiety

  • Cancer

  • Pain Management

  • And More

Personalized Support and Services

 

The program focuses on creating a comprehensive care plan built in collaboration with you to ensure your specific needs are met. Participants receive:

 

 

  • Personalized Attention: Continuous support from a dedicated care manager to maintain health and wellness.

 

  • Regular Communication: Phone check-ins between scheduled office visits to keep your health on track.

 

  • Medication Guidance: Education and management for your prescriptions.

 

  • Easy Access: Care Managers availability for questions regarding symptoms, medications, or prescription refills.

 

  • Appointment Coordination: Tracking for visits with primary physicians, specialists, surgeons, and therapy services.

Comprehensive Resource Connection

We believe in helping the whole person by offering resources that go beyond just medical care. Our team can help you with:

 

  • Transportation to doctor appointments and other travel needs.

 

  • Food and nutrition needs, as well as other basic needs.

 

  • Finding housing or shelter and connecting with resources for housing assistance.

 

  • Personal finance training and resources, and support for family members who are caring full-time for a loved one.

 

  • Help with managing medications through Valley View Discount Pharmacy and other local pharmacies.

Eligibility
 

You are eligible for the CCM program if you meet the following criteria:

 

  • You have Medicare or a Medicare Advantage plan.

 

  • You are currently managing two or more long-term health conditions.

Enrollment

​

​

Three Steps to Get Started:

 

  • Schedule an Appointment: Visit your Primary Care Provider as soon as possible.

 

  • Express Your Interest: Inform your provider during the visit that you would like to enroll in Chronic Care Management services.

 

  • Meet Your Coordinator: Your provider will submit a referral, and a CCM Coordinator will contact you to complete the enrollment process.

What is FTCA?

Federal Tort Claims Act

Under the Federally Supported Health Centers Assistance Acts of 1992 and 1995, health centers that receive funds through the Health Center Program may be deemed as employees of the Public Health Service (PHS) for purposes of Federal Tort Claims Act (FTCA) coverage for the performance of medical, surgical, dental, and related functions within the scope of employment. The Health Center FTCA Program increases the availability of funds to health centers to provide services within the Health Center Program scope of project by reducing or eliminating health centers' spending on malpractice insurance premiums for such services.

What services are covered under the FTCA Program? Deemed Health Center Program grantees are immune from medical malpractice lawsuits resulting from the performance of medical, surgical, dental, or related functions within the approved scope of project.
