Understanding Your Needs Through a Behavioral Health Assessment

 

Empowerment begins with understanding your story.

​

 

Based on a Behavioral Health Assessment to identify the mental, emotional, and situational challenges affecting your daily life—such as anxiety, depression, PTSD, or substance use concerns--you may be eligible for enrollment into the ongoing case management supportive services.

​

 

This assessment helps us clearly understand all barriers that may be holding you back. Once enrolled, you will work closely with a Case Manager who collaborates with our Mental Health Providers to ensure you receive personalized and coordinated care.

​

 

Together, we build a plan that supports your journey to becoming stronger, healthier, and more self-sufficient.

Our Case Managers, Behavioral Healthcare Providers, and staff walk beside each client with zero judgment—no matter your circumstances or past experiences.

 

Your information and everything you share is completely confidential and fully protected under HIPAA regulations.

Who Qualifies?

 

Patients who receive Medicaid coverage for behavioral health services as stated by the Ohio Administrative Code.

  • Helping you handle symptoms or struggles related to mental health.

Every empowerment plan is tailored to you.

Understanding an Individualized Service Plan (ISP)

 

An Individualized Service Plan (ISP) is a personalized care plan created collaboratively with you, focusing on your goals and challenges. It outlines steps for improving your mental health and daily life. Mandated by the State of Ohio, the ISP ensures timely and appropriate support.

Your ISP typically includes:

 

  • Goals (e.g., reducing anxiety, improving relationships)

  • Services provided (case management, crisis support)

  • Meeting frequency with the case manager or care team

  • Service locations (office, home, community, phone/video)

  • Involved parties (you, case manager, behavioral health provider)

  • Progress measurement methods (check-ins to assess effectiveness)

What is FTCA?

Federal Tort Claims Act

Under the Federally Supported Health Centers Assistance Acts of 1992 and 1995, health centers that receive funds through the Health Center Program may be deemed as employees of the Public Health Service (PHS) for purposes of Federal Tort Claims Act (FTCA) coverage for the performance of medical, surgical, dental, and related functions within the scope of employment. The Health Center FTCA Program increases the availability of funds to health centers to provide services within the Health Center Program scope of project by reducing or eliminating health centers' spending on malpractice insurance premiums for such services.

What services are covered under the FTCA Program? Deemed Health Center Program grantees are immune from medical malpractice lawsuits resulting from the performance of medical, surgical, dental, or related functions within the approved scope of project.
